when do we use the passé simple and not the imparfait ?

  If in Exer the x stands for any other letter, that would include eler and eter, so why does the heading expressly state them?

In the sentence from the lesson which reads-

“Vous allez aux Etats-Unis.” Here the acute accent on ‘E’ is missing .  Should it not be as “États-Unis”?

How come in the 3rd sentence, "l'avion" is used and in the rest of the excerpt, "le vol" is used?
