No, this is simple infinitif présent.

Elle est soulagée de ne pas devoir être opérée. -- She is relieved not having to be operated.
Elle est soulagée de ne pas avoir dû être opérée. -- She is relieved not having had to be operated.

The second sentence uses the infinitif passé. Maybe you are confused by the second infinitive (être) following right after the first one (devoir)?
